  • Discussion about all of your treatment options: lifestyle, non-hormonal, and hormonal

--

  • Prescriptions (if that’s what we decide) to treat your bothersome symptoms: non-hormonal and hormonal including Menopause Hormone Therapy (MHT) and other medications as indicated

--

  • The Midlife Health Review to discuss your risks for disease and decline, and how you can prolong your health and independence through lifestyle modifications, supplements, and medications

--

  • Visit Summary: a written summary will be provided to you including what you're already doing well, what you can nurture, medications we started and why, helpful resources, and next steps

THE FOLLOW-UP

Judy will see you in follow-up to ensure that your plan of care and prescription therapy is going well.

  • Reassessment: if we start any medications, we follow-up to ensure they are working and without bothersome side effects, and make any necessary adjustment. We will also follow-up on the non-prescription recommendations to see how you're doing and if you have any questions. Note that more than one follow-up may be required if we adjust medications.

--

  • Annual Visits per guideline recommendations if you are on Menopause Hormone Therapy to ensure benefit still outweighs risk and to update prescriptions

--

  • Shared Care: as more providers get trained in Perimenopause and Menopuase care, there will be increasing opportunity to return care to your Primary Care team for OHIP-covered care (or until care becomes more NP-inclusive). We will evaluate this to ensure you continue to receive guideline-recommended care.
